Body Ache
Generalised aching across the body — common causes from viral illness and tension to vitamin D deficiency and metabolic issues, and how it is assessed.

Overview
A generalised body ache — a feeling of soreness or aching across the muscles and sometimes the joints — is very common and usually part of a viral illness, overexertion, stress, poor sleep, or a metabolic issue such as vitamin D deficiency. It is a non-specific symptom, so the aim of assessment is to tell a simple, self-limiting cause from one that needs investigation. This page focuses on generalised aching; localised joint pain is on the Joint Pain page and muscle pain on the Muscle Pain page.
Common causes
- Viral infections — flu, dengue and other febrile illnesses (see Fever & Infectious Diseases).
- Overexertion or unaccustomed activity.
- Tension, stress and poor sleep.
- Vitamin D deficiency — a common cause of vague generalised aching.
- Thyroid disorders — an underactive thyroid can cause aching (see Thyroid Disorders).
- Anaemia and chronic illness — can cause aching with fatigue.
- Medicines — such as statins.
- Fibromyalgia and chronic pain syndromes in some people (see Chronic Musculoskeletal Pain).
When body ache needs assessment
A short-lived ache with a clear cause (a viral illness or overexertion) usually needs no tests. Aching that is persistent, severe, accompanied by fever, weight loss, joint swelling, weakness, or other new symptoms needs assessment. The pattern of associated symptoms guides which tests are chosen.
Evaluation and investigations
Evaluation is guided by the story and may include blood counts, inflammatory markers (ESR, CRP), vitamin D, calcium, thyroid function, blood sugar, and a creatine kinase level when muscle involvement is suspected. A medicine review is essential. See Abnormal Test Results.
Management overview
Management is cause-specific — rest and fluids for a viral illness, treating an underlying condition, correcting vitamin D or thyroid deficiency, reviewing and adjusting culprit medicines, and addressing sleep and stress. For persistent aching without a clear cause, a structured assessment for chronic pain syndromes may be appropriate. See Vitamin D Deficiency and Bone Health and Chronic Musculoskeletal Pain.
Lifestyle and activity
Regular activity, good sleep, stress management, adequate hydration, and a balanced diet help the common, non-specific causes of body ache. Gradual return to activity after a viral illness prevents prolonged fatigue.
Warning signs — seek prompt care
- Body ache with high fever, severe headache or rash — needs prompt assessment.
- Ache with marked weakness or dark urine — emergency.
- Persistent aching with unexplained weight loss or night sweats.
