Fatigue, Weakness & General Symptoms
Night Sweats
Drenching sweats at night — causes from a warm room and hormonal changes to infections and other underlying conditions, the evaluation, and when they need prompt attention.

Overview
Night sweats are episodes of sweating during sleep that are heavy enough to soak nightclothes or bedding. A warm bedroom, too many blankets, or a mild feverish illness can cause harmless sweating at night, but true drenching night sweats that occur repeatedly — particularly with fever, weight loss or other symptoms — need medical evaluation, because they can be a sign of an underlying infection, a thyroid or hormonal cause, diabetes, or a less common inflammatory or other condition. Occasional mild night sweating is common; persistent heavy night sweats are the concern.
Common causes
- A warm bedroom or too much bedding — the commonest harmless cause.
- A viral or bacterial infection, including tuberculosis.
- An over-active thyroid (hyperthyroidism).
- Diabetes with low blood sugar overnight (hypoglycaemia).
- Hormonal changes, including menopause.
- Anxiety and stress.
- Medication effects, including antidepressants and fever-reducing medicines.
- Chronic inflammatory or rheumatological conditions.
- Less commonly, an underlying serious condition such as a blood disorder or malignancy.
Symptoms and associated features
The hallmark is repeated, heavy night-time sweating. Associated features guide the work-up: fever and weight loss suggest infection; palpitations and heat intolerance suggest hyperthyroidism; early-morning sweating, hunger or shakiness suggest overnight hypoglycaemia; and joint pains or persistent fatigue suggest an inflammatory cause. Whether the sweats are new, changing, or long-standing is important.
Evaluation and investigations
Evaluation reviews the sleep environment, the pattern of sweats, and associated symptoms, and uses targeted tests: complete blood count, inflammatory markers (ESR, CRP), blood sugar or HbA1c, thyroid function tests, kidney and liver function, a urine routine, and a chest X-ray when infection is suspected. Further tests are arranged by the pattern. See Fever & Infectious Diseases, Thyroid Disorders, Diabetes & Metabolic Health and Abnormal Test Results.
Management overview
Management treats the underlying cause. Harmless environmental causes are resolved with simple bedroom adjustments; an infection is treated; an over-active thyroid is managed; overnight hypoglycaemia is addressed by adjusting diabetes treatment; hormonal and medication-related causes are reviewed; and inflammatory or less common causes are managed specifically with specialist input where needed.
Follow-up and monitoring
Follow-up confirms the night sweats have settled and any abnormal results have normalised. Persistent night sweats despite treatment are reviewed for less common causes and referred for specialist assessment when appropriate.
Warning signs — seek prompt care
- Drenching night sweats with persistent fever and weight loss.
- Night sweats with a persistent cough or coughing up blood.
- Night sweats with markedly enlarged lymph nodes.
- Night sweats with severe sweating, shakiness and confusion overnight — possible hypoglycaemia.
