Weakness

Generalised weakness vs sudden focal neurological weakness — distinguishing everyday causes from urgent warning symptoms, and the right next steps.

Overview

Weakness is a common symptom with two very different meanings. Generalised weakness — feeling tired, heavy or lacking energy — is usually part of fatigue and has many treatable medical causes. Focal neurological weakness — sudden loss of strength in one side, limb or the face — can be a stroke and is an emergency. Telling these apart is critical. This page covers both; for generalised tiredness in depth, see Fatigue, Weakness & General Symptoms.

Generalised weakness — common and usually non-urgent

  • Anaemia.
  • Thyroid dysfunction (underactive thyroid).
  • Uncontrolled or poorly controlled diabetes.
  • Vitamin D or B12 deficiency.
  • Dehydration, recent infection or poor sleep.
  • Medication side effects.
  • Prolonged inactivity or deconditioning.

Focal neurological weakness — urgent

Sudden weakness on one side of the body, in one limb, or in the face — especially with slurred speech, visual loss or confusion — can indicate a stroke and needs emergency care immediately. The same applies to weakness that is rapidly progressing or accompanied by numbness, difficulty walking or loss of bowel/bladder control.

Evaluation

For generalised weakness, evaluation includes blood tests for anaemia, thyroid, blood sugar, vitamins and electrolytes, a medication review and a general examination. For any focal or sudden weakness, urgent assessment and imaging are arranged without delay.

When to seek emergency care

  • Sudden one-sided weakness of the face, arm or leg.
  • Weakness with slurred speech, confusion or visual loss.
  • Rapidly progressing weakness or difficulty walking.
  • Weakness after a fall or head injury.
  • Weakness with loss of bowel or bladder control.
