Severe Headache

Severe headache — distinguishing a primary severe headache (migraine, cluster) from a sudden thunderclap or other emergency that needs immediate care.

Overview

Severe headache can be alarming, and rightly so — but most severe headaches are still primary (migraine or cluster). What matters most is recognising the patterns that signal an emergency and need immediate assessment. This page helps you tell the difference. For specific severe primary headaches, see Migraine and Cluster Headache.

Severe but usually primary

  • Migraine — severe one-sided throbbing pain with nausea, building over hours.
  • Cluster headache — severe one-sided eye pain in short, repeated attacks with eye watering.
  • A familiar severe headache that matches your usual pattern.

Severe and potentially an emergency

  • Thunderclap headache — sudden, maximal within one minute; “worst headache of my life”. This can indicate bleeding around the brain (subarachnoid haemorrhage) and needs emergency care.
  • Severe headache with fever, stiff neck, rash or confusion — possible meningitis.
  • Severe headache with new weakness, numbness, slurred speech or visual loss — possible stroke.
  • Severe headache after a head injury.
  • A first severe headache after age 50, or with scalp tenderness and jaw pain on chewing — possible temporal arteritis, which risks sight loss.

What to do

Any thunderclap headache, or severe headache with the emergency features above, warrants immediate emergency assessment — do not wait for a routine appointment. A severe headache that matches your known migraine or cluster pattern can be managed with your usual treatment, but a clear change from your usual pattern should be reviewed promptly.
