Joint Swelling

A swollen joint — when it is fluid from wear, when it signals gout or infection, and how the pattern of swelling guides prompt assessment.

Overview

A swollen joint is a common reason for assessment. Swelling may be a slow accumulation of fluid in a worn joint (osteoarthritis), a sign of an inflammatory arthritis, an acute crystal attack (gout), or — importantly — a sign of joint infection, which needs urgent care. This page focuses on the joint; generalised leg swelling has a different set of causes covered on Swelling & Oedema.

Patterns of swelling

  • Slow, cool swelling of a worn joint that comes and goes — typical of osteoarthritis.
  • Warm, swollen joints with prolonged morning stiffness — inflammatory arthritis.
  • Sudden, hot, red, very tender swelling of a single joint, often the big toe — gout (see Gout Attacks).
  • A single hot, swollen, very painful joint with fever — possible infection, urgent.
  • Swelling after an injury — suggests bleeding, meniscus or ligament injury.

Why a single hot joint needs prompt assessment

A single hot, swollen, very tender joint can be gout, pseudogout, or a joint infection. Because infection can destroy a joint within hours, a genuinely hot joint with fever needs same-day assessment, sometimes with joint-fluid analysis. It should not be assumed to be “just gout” without assessment.

Evaluation and investigations

Evaluation combines examination (warmth, tenderness, range of movement) with selected tests — blood counts, inflammatory markers (ESR, CRP), uric acid, and an X-ray. Joint-fluid analysis is arranged when infection or crystals are suspected. Blood tests for gout and kidney function are linked — see High Uric Acid and Kidney Health.

Management overview

Management is cause-specific — activity modification and careful pain relief for osteoarthritic fluid, anti-inflammatory and disease-modifying treatment for inflammatory arthritis, anti-inflammatory treatment and uric-acid lowering for gout, and urgent orthopaedic input for infection. See Osteoarthritis, Rheumatoid Arthritis and Gout.

Warning signs — seek prompt care

  • A single hot, red, very swollen joint with fever — urgent.
  • Joint swelling after a significant injury with inability to bear weight or move the joint.
  • Rapidly worsening swelling with severe pain.
