Overview

Fatigue is a persistent sense of tiredness, exhaustion or lack of energy that is out of proportion to activity and not relieved by rest. It is one of the commonest symptoms in general medicine and affects daily work, concentration and quality of life. Fatigue is a symptom, not a diagnosis: the same feeling can come from poor sleep or stress, or from a treatable medical condition such as anaemia, a thyroid disorder, diabetes, an infection, a vitamin deficiency, or a medication side-effect. This is the main page for general tiredness and low energy; fatigue that has lasted many weeks is explored further on Chronic Fatigue.

Common causes

Symptoms and associated features

The hallmark is tiredness that persists despite rest and interferes with daily life. Associated features guide the cause: breathlessness and pallor suggest anaemia; weight gain and cold intolerance suggest hypothyroidism; thirst, weight loss and frequent urination suggest diabetes; low mood and poor interest suggest a mood cause; snoring and unrefreshing sleep suggest a sleep disorder. A clear description of the pattern — onset, duration, fluctuations, and what helps or worsens it — is the most useful part of the assessment.

Evaluation and investigations

Evaluation combines a careful history and examination with targeted tests. A typical baseline panel includes a complete blood count (CBC), iron studies and ferritin, vitamin B12 and folate, vitamin D, blood sugar or HbA1c, thyroid function tests (TSH, free T4), kidney and liver function tests, electrolytes, and a urine routine. An ECG is arranged when breathlessness or palpitations are present. Tests are chosen by the pattern rather than ordered wholesale. See Anaemia & Blood Problems, Thyroid Disorders, Diabetes & Metabolic Health and Abnormal Test Results.

Management overview

Management treats the identified cause: correcting anaemia and deficiencies, balancing the thyroid, controlling blood sugar, treating infection, replacing vitamin D, addressing a sleep disorder, and reviewing medicines. When no medical cause is found, structured attention to sleep, nutrition, activity, stress and mood — with planned follow-up — is more effective than repeated unplanned testing. The aim is to find and treat the cause, not to mask the tiredness.

Follow-up and monitoring

Follow-up confirms the symptom is improving and that any abnormal results have normalised with treatment. Persistent fatigue despite a normal baseline is reviewed for less common causes and, where appropriate, referred for specialist assessment. See Preventive Health & Health Check-ups.

Warning signs — seek prompt care

  • Fatigue with chest pain, breathlessness at rest or fainting — urgent.
  • Fatigue with unintended weight loss, fever or night sweats.
  • Sudden severe fatigue with pallor — possible severe anaemia.
  • Fatigue with confusion or drowsiness — urgent.
