Fever & Infectious Diseases
Acute Fever
A fever of recent onset (a few days) — usually viral, how to recognise the pattern, when to investigate, and when it is safe to manage at home.

Overview
Acute fever means a fever that has come on recently and lasted a few days. The large majority are caused by common viral infections and settle on their own with rest and fluids. The key tasks are to gauge how unwell the person is, watch for warning signs, and decide whether tests or specific treatment are needed. This page covers the acute pattern; for the general concept see Fever, and for fevers that keep returning see Recurrent Fever.
Typical features
When to investigate an acute fever
- Fever beyond 3–4 days without a clear cause or improvement.
- Very high fever or fever with chills and sweating (consider malaria).
- Fever with rash, severe headache or body pain (consider dengue).
- Fever with diarrhoea and vomiting (consider gastrointestinal infection).
- Fever with cough and breathlessness (consider pneumonia or COVID-19).
- People with diabetes, pregnancy, low immunity or other long-term conditions.
Common investigations
Tests are chosen based on the suspected cause rather than ordered routinely: a complete blood count, malaria smear, dengue NS1/IgM, urine routine, and where indicated Widal or COVID-19 testing. Dr. Nayak avoids unnecessary panels and antibiotics.
Management
Simple viral acute fever is managed with rest, hydration and fever-reducing medicine. Specific treatment is started once a cause is identified — antimalarials for malaria, fluids and monitoring for dengue, antibiotics for confirmed bacterial infection. Warning signs require urgent care.
Warning signs — seek urgent care
- Breathlessness, chest pain or bluish lips.
- Bleeding, rash, or blood in vomit or stools.
- Severe headache with stiff neck, confusion or drowsiness.
- Persistent vomiting, severe abdominal pain, or reduced urine output.
- Very low blood pressure or seizures.
