Fever with Body Ache
Fever with muscle and joint aches — very common in viral fever, dengue and flu, how to tell them apart, and when to investigate.

Overview
Body ache with fever is one of the most common combinations and is usually part of a viral illness. Severe muscle and joint pain, however, is a hallmark of dengue (sometimes called “breakbone fever”) and influenza. This page covers the symptom combination and how it guides the evaluation; for the specific diseases see Viral Fever, Dengue Fever and Influenza / Flu.
Common causes
- Viral fever — generalised aches and tiredness.
- Dengue — severe muscle and joint pain, high fever, headache, rash.
- Influenza (flu) — abrupt fever with prominent body ache and weakness.
- Malaria and typhoid — can cause body ache alongside other features.
- Other viral illnesses including COVID-19.
Evaluation
Evaluation looks at the onset, severity and pattern of aches alongside fever. Dengue is suspected when severe pain is paired with high fever, headache and rash — prompting a complete blood count (platelets), dengue NS1/IgM and haematocrit. Flu and COVID-19 testing are considered when respiratory symptoms are present.
Management
Most viral causes are managed with rest, hydration and fever-reducing medicine. Dengue needs careful fluid balance and daily platelet/haematocrit monitoring. Symptomatic relief for aches is provided while the underlying cause is addressed. Avoid aspirin and NSAIDs when dengue is possible, due to bleeding risk — use paracetamol as advised.
Warning signs — seek urgent care
- Severe pain with bleeding, rash, or abdominal pain (possible dengue).
- Breathlessness, chest pain or confusion.
- Reduced urine output or very low blood pressure.
