Fever with Chills
Fever accompanied by shivering and sweating — a pattern that often points to malaria or bacterial infection, and what the evaluation involves.

Overview
Chills — a sensation of cold with shivering, often followed by sweating as the fever peaks — are a specific feature that helps narrow the cause. The classic pattern of chills and rigors followed by high fever and sweating is typical of malaria, but chills also occur in other bacterial infections and in some viral illnesses. This page covers the symptom combination; for the underlying diseases see Malaria and the other infection pages.
Common causes
- Malaria — the classic cause of fever with chills and rigors.
- Bacterial infections — urinary tract infection, pneumonia, blood infection.
- Dengue and other viral fevers — can cause chills, though less typically.
- Abscess or localised infection.
- Certain medicines or reactions (less common).
Evaluation
Evaluation focuses on the fever pattern, mosquito exposure, and accompanying symptoms. A malaria smear (peripheral blood film) and antigen test are done when malaria is suspected, along with a complete blood count, urine routine and, where indicated, blood culture and other tests.
Management
Treatment is cause-specific: antimalarials for confirmed malaria, targeted antibiotics for bacterial infection, and supportive care for viral illness. Chills themselves are managed with warmth and fever-reducing medicine while the underlying cause is treated.
Warning signs — seek urgent care
- Very high fever with chills and confusion or drowsiness.
- Chills with very low blood pressure or reduced urine output (possible sepsis).
- Severe headache, breathlessness, or bleeding.
