Fever with Rash

Fever with a skin rash — dengue and other viral illnesses, when a rash with fever needs urgent assessment, and what the evaluation involves.

Overview

A rash with fever can be a harmless feature of a viral illness or a sign of a more serious infection. Dengue classically causes a flushed appearance and a rash, and several other viral infections produce characteristic rashes. Some rashes with fever — particularly those that do not fade on pressure, or that appear with bleeding — need urgent assessment. This page covers the combination; for skin conditions in general see Skin Problems.

Common causes

  • Dengue — flushed skin, a measles-like rash, and sometimes tiny red spots (petechiae).
  • Other viral illnesses — various rashes, mostly harmless.
  • Typhoid — faint rose-coloured spots on the trunk in some cases.
  • Medicine reactions — rash with fever after starting a new drug.
  • Serious bacterial infection — rash that does not fade on pressure (emergency).

Warning signs — seek urgent care

  • A rash that does not fade when you press a clear glass against it.
  • Rash with bleeding gums, nosebleeds, or blood in vomit or stools (possible dengue).
  • Rash with very high fever, drowsiness or low blood pressure.
  • A rapidly spreading rash with severe illness.

Evaluation

Evaluation examines the rash type and distribution alongside the fever pattern. When dengue is suspected, a complete blood count (platelets), dengue NS1/IgM and haematocrit are ordered. A rash that does not fade on pressure or any bleeding rash needs urgent hospital assessment.

Management

Management depends on the cause: supportive care and monitoring for dengue, stopping a suspected offending medicine for drug reactions, and urgent hospital care for serious bacterial infection or bleeding rashes.
