Fever with Cough
Fever with a cough — points to a respiratory infection such as flu, COVID-19 or pneumonia, when to investigate, and the warning signs.

Overview
Fever with a cough usually means a respiratory infection — most often a viral cold, influenza or COVID-19, and sometimes pneumonia or bronchitis. Most settle with supportive care, but breathlessness, chest pain or a persistent cough need assessment. This page covers the combination; for breathing problems in depth see Respiratory & Breathing Problems.
Common causes
- Common cold and viral upper respiratory infection.
- Influenza (flu) — abrupt fever, cough, body ache.
- COVID-19 and other respiratory viral illness.
- Acute bronchitis.
- Pneumonia — fever, cough with phlegm, breathlessness; may need antibiotics.
- Tuberculosis — persistent cough (sometimes with blood), low-grade fever, weight loss.
Evaluation
Evaluation reviews the cough type (dry vs productive), duration, and breathing. A chest examination is performed; a chest X-ray is ordered when pneumonia or tuberculosis is suspected, along with relevant blood tests and, where indicated, COVID-19 testing or sputum examination.
Management
Most viral respiratory infections are managed with rest, hydration and symptomatic care. Pneumonia may need antibiotics. Tuberculosis requires specific, prolonged treatment. Breathlessness or low oxygen need urgent hospital care.
Warning signs — seek urgent care
- Breathlessness, chest pain or bluish lips.
- Very high fever with confusion or drowsiness.
- Cough with significant blood.
- Persistent cough and fever beyond a week, or worsening despite care.
