Overview

Obesity is a chronic medical condition in which excess body fat has accumulated to a level that harms health. It is defined by a body mass index (BMI) of 30 or above, though for South Asian adults the health risks begin at a lower BMI, so the assessment is always interpreted in context. Obesity is not a failure of willpower — it is a complex condition shaped by biology, genetics, environment, lifestyle and, in some cases, underlying disease. It is the main disease-focused page for this service; Overweight, Abdominal Obesity, BMI and Weight Assessment and Medical Weight Management each cover a specific angle and link here.

How obesity is defined and assessed

BMI gives the broad category (30–34.9 class I, 35–39.9 class II, 40 or above class III), while waist circumference shows how much harmful abdominal fat is present. Because BMI does not distinguish muscle from fat or say where fat is carried, both measures are used together, and the trend over time matters more than any single reading. See BMI and Weight Assessment for how the numbers are interpreted.

Causes and contributors

  • A diet high in processed foods, refined carbohydrates, sugary drinks and large portions.
  • A sedentary lifestyle and low everyday activity.
  • Inadequate or poor-quality sleep and chronic stress.
  • Genetic and family factors that influence appetite, metabolism and fat storage.
  • Certain medicines, such as some antidepressants, antipsychotics, steroids and diabetes medicines.
  • Underlying medical conditions in some cases — see Obesity and Thyroid Disorders.
  • Age-related changes in metabolism and muscle mass.

How obesity affects health

Excess body fat, especially around the abdomen, raises the risk of type 2 diabetes, high blood pressure, abnormal cholesterol and triglycerides, fatty liver, heart disease and stroke, certain cancers, sleep-disordered breathing and breathlessness, and osteoarthritis of the weight-bearing joints. These risks tend to cluster as the metabolic syndrome and multiply rather than simply add. Even a modest, sustained weight loss of 5–10% of body weight can meaningfully reduce these risks.

Evaluation and investigations

Evaluation includes a detailed history of weight change, diet, activity, sleep, stress and medicines; measurement of weight, height, BMI and waist circumference; blood pressure; and selected tests — blood sugar or HbA1c, lipid profile, thyroid function, liver function, kidney function and a complete blood count — to identify metabolic consequences and any underlying cause. Further hormonal tests are ordered when the history suggests them.

Management overview

Management combines a sustainable eating pattern, increased physical activity, sleep and stress attention, and behaviour change as the foundation. Where there is meaningful health risk and lifestyle alone is not enough, medical treatment may be appropriate alongside lifestyle change. The goal is a realistic, individualised target and a plan you can sustain long-term — not an unrealistic number. See Medical Weight Management and Healthy Weight Loss.

Follow-up and monitoring

Weight, waist circumference, blood pressure and the metabolic markers are monitored over time. Regular follow-up catches plateaus and small regains early and adjusts the plan. See Weight Loss Plateau and Weight Regain.

Warning signs — seek prompt care

  • Breathlessness, chest pain or fainting with exertion.
  • Severe sleep-related breathing concerns with loud snoring and daytime sleepiness.
  • Rapid weight gain with swelling of the legs or face.
  • Weight gain with marked fatigue, cold intolerance or other hormonal symptoms.
