Difficulty Losing Weight

When genuine effort is not producing results — the medical, metabolic and behavioural reasons weight loss can be hard, and how a structured review helps.

Overview

Difficulty losing weight is one of the commonest frustrations people describe. It usually means that despite changes to eating and activity, the weight is not coming down as expected — or comes down and then stops. This page focuses on why that happens and what a medical review can add; the structured plan itself is covered on the Medical Weight Management page, and the specific stall after initial success on the Weight Loss Plateau page.

Why weight loss can be hard

  • Underestimating intake — portions, snacks, cooking oils and drinks add up more than we realise.
  • Overestimating activity — exercise burns less than is often assumed.
  • Insulin resistance and high insulin levels, which favour fat storage.
  • An underactive thyroid slowing the metabolic rate (see Obesity and Thyroid Disorders).
  • PCOS and other hormonal contributors in some women.
  • Poor sleep and chronic stress, which raise hunger and cravings.
  • Certain medicines that promote weight gain.
  • The body’s biological response to weight loss — hunger rises and metabolism adapts.
  • Loss of muscle mass with age or very restrictive diets, lowering energy use.

Evaluation

A review looks honestly at the eating and activity pattern (often with a food and activity diary), checks for medical contributors with selected tests (blood sugar, HbA1c, thyroid function, lipids), reviews medicines, and assesses sleep and stress. The aim is to find the specific reason progress has stalled for you, rather than assuming a lack of effort.

Management overview

Management is directed at the reason found — correcting an underactive thyroid, addressing insulin resistance, adjusting the eating and activity plan realistically, improving sleep and stress, and reviewing medicines. Where appropriate, medical treatment is considered alongside lifestyle change. See Medical Weight Management and Weight Loss Plateau.
