Obesity & Weight Management
Obesity and Hypertension
How excess weight raises blood pressure, why weight loss lowers it, and how weight and blood-pressure care are managed together.

Overview
Excess weight is one of the commonest contributors to high blood pressure. It raises blood pressure through several mechanisms — increased sympathetic activity, activation of hormonal systems that retain salt and water, and physical compression of the kidneys by abdominal fat. The relationship is reversible: weight loss is one of the most effective non-medication ways to lower blood pressure. This page focuses on the relationship between the two; the blood-pressure condition itself is on the High Blood Pressure and Hypertension pages, and the weight plan on Medical Weight Management.
How obesity raises blood pressure
More body tissue means the heart must pump more blood, raising the pressure; visceral fat activates systems (including the renin-angiotensin-aldosterone system) that retain salt and water; and sleep-disordered breathing, which is more common with obesity, further raises blood pressure. The result is often blood pressure that is harder to control with medication alone until weight is addressed.
Why weight loss helps
A modest, sustained weight loss can meaningfully lower blood pressure, reduce the number and dose of blood-pressure medicines needed, and improve their effectiveness. Salt reduction and the dietary pattern that supports weight loss also lower blood pressure directly. See High Blood Pressure.
Managing the two together
Effective care treats weight and blood pressure together — a sustainable low-salt eating pattern, activity, medication where appropriate, and regular monitoring of both blood pressure and weight. Some blood-pressure medicines are preferred in people with weight-related metabolic risk. See Hypertension & Cardiovascular Risk and Medical Weight Management.
