Overview

Fever with a sore throat is most often viral and settles with supportive care. A bacterial throat infection (strep throat) needs antibiotics and can, rarely, lead to complications. Difficulty swallowing, drooling, or a muffled voice can indicate a more serious throat infection needing urgent care. This page covers the combination; for ongoing breathing concerns see Respiratory & Breathing Problems.

Common causes

  • Viral pharyngitis — the most common; often with cold symptoms.
  • Influenza and COVID-19 — sore throat with fever and cough.
  • Strep throat (bacterial) — fever, severe sore throat, swollen tonsils with pus, swollen neck glands.
  • Tonsillitis — recurrent sore throat with fever.
  • Peritonsillar abscess — severe one-sided throat pain, difficulty swallowing, muffled voice (urgent).

Evaluation

Evaluation examines the throat, tonsils and neck glands. A strep swab or rapid test may be used when bacterial infection is suspected. Blood tests and a throat swab are ordered when needed. Difficulty swallowing or breathing prompts urgent assessment.

Management

Viral sore throat is managed with rest, warm fluids, salt-water gargles and fever-reducing medicine. Strep throat is treated with antibiotics. Severe throat infections or abscess need urgent hospital care.

Warning signs — seek urgent care

  • Difficulty swallowing saliva, drooling, or a muffled voice.
  • Difficulty breathing or noisy breathing.
  • Very high fever with severe one-sided throat pain.
  • Persistent fever and sore throat beyond several days despite care.
