Cough with Phlegm

A productive cough bringing up phlegm — what the colour and amount mean, common causes from bronchitis to pneumonia, and when it needs tests.

Overview

A productive cough is one that brings up phlegm (sputum). The body produces phlegm to trap and clear irritants and infection, so a productive cough often points to an infection or ongoing airway inflammation. The amount, colour and thickness of the phlegm, and how long it has lasted, help tell a simple viral bronchitis from pneumonia, chronic bronchitis or another cause. This page covers the productive pattern; for a cough without phlegm see Dry Cough, and for the central overview see Cough.

Common causes

  • Acute bronchitis — often follows a viral infection; phlegm may be clear or coloured.
  • Pneumonia — coloured or rusty phlegm with fever and breathlessness.
  • Chronic bronchitis and COPD — daily productive cough, especially in smokers.
  • Post-nasal drip draining into the chest.
  • Tuberculosis — persistent phlegm, sometimes with blood, weight loss and night sweats.
  • Asthma with mucus production.

What the phlegm can suggest

Clear or white phlegm is common in viral illness and allergies. Yellow or green phlegm can occur with both viral and bacterial infections and does not by itself prove a bacterial infection. Rusty or blood-tinged phlegm raises concern for pneumonia or tuberculosis. Persistently blood-stained phlegm always needs prompt assessment.

Evaluation

Evaluation reviews the duration, phlegm character, fever, breathlessness and smoking history. A chest examination is performed, and a chest X-ray is ordered when pneumonia or tuberculosis is suspected. Sputum examination and culture, and blood tests, are ordered when a bacterial infection or tuberculosis is likely.

Management

Management treats the cause. Viral bronchitis is managed supportively. Pneumonia and bacterial infections are treated with targeted antibiotics. COPD and chronic bronchitis are managed with bronchodilators, smoking cessation and vaccinations. Tuberculosis requires specific prolonged treatment. Hydration helps thin the phlegm and ease clearance.

Warning signs — seek urgent care

  • Blood in the phlegm.
  • High fever with breathlessness or confusion.
  • Chest pain or bluish lips.
  • Large amounts of phlegm or worsening despite treatment.
