Chronic Fatigue

Fatigue that has lasted weeks or months — the structured evaluation for persistent tiredness, the common and less common causes, and when further assessment is needed.

Overview

Chronic fatigue is tiredness that has persisted for several weeks or months rather than settling within days. While short-term tiredness from poor sleep, a viral illness or overwork usually resolves with rest, fatigue that carries on deserves a structured evaluation because it often points to an underlying medical condition that can be treated. This page focuses on the persistent pattern; the general overview of tiredness and low energy is on the Fatigue page, and tiredness that simply will not go away is the same concern as “persistent tiredness”.

Common causes of persistent fatigue

  • Anaemia and nutritional deficiencies (iron, vitamin B12, folate, vitamin D).
  • Thyroid disorders, especially hypothyroidism.
  • Diabetes and prediabetes.
  • Chronic or recurrent infections.
  • Sleep disorders, including obstructive sleep apnoea and insomnia.
  • Long-term medication effects.
  • Chronic kidney, liver or digestive disease.
  • Chronic respiratory conditions.
  • Inflammatory and rheumatological conditions.
  • Mood disorders — anxiety and depression.

Symptoms and associated features

The defining feature is duration — tiredness present for weeks or months. Associated features guide the work-up: breathlessness points to anaemia; weight changes and temperature intolerance point to the thyroid or diabetes; snoring and unrefreshing sleep point to a sleep disorder; joint pains and morning stiffness point to an inflammatory cause; and low mood or loss of interest point to a mood disorder. A record of how the fatigue has changed over time is particularly helpful.

Evaluation and investigations

Chronic fatigue is evaluated with a structured history, examination and a targeted test panel: complete blood count, iron studies and ferritin, vitamin B12, folate and vitamin D, blood sugar or HbA1c, thyroid function tests, kidney and liver function, electrolytes, inflammatory markers (ESR, CRP) where indicated, and a urine routine. A sleep assessment is arranged when sleep apnoea is suspected. Tests are targeted to the pattern rather than ordered in an unguided bundle. See Abnormal Test Results and Preventive Health & Health Check-ups.

Management overview

Management treats the cause the evaluation identifies. Deficiencies are corrected, the thyroid is balanced, diabetes is controlled, infection is treated, sleep is addressed, and medicines are reviewed. When the baseline evaluation is normal, attention turns to sleep, nutrition, activity, stress and mood, with a graded return to activity and planned follow-up rather than repeated unplanned testing. Persistent fatigue despite a normal work-up is reviewed for less common causes and referred for specialist assessment when appropriate.

Follow-up and monitoring

Follow-up reviews the symptom response and any abnormal results over time. A structured plan catches relapse or new features early and avoids repeated courses of unplanned investigation.

Warning signs — seek prompt care

  • Chronic fatigue with unintended weight loss, fever or night sweats.
  • Fatigue with chest pain, breathlessness at rest or fainting.
  • New confusion, drowsiness or a marked change in consciousness.
  • Fatigue that is progressively worsening rather than fluctuating.
