Frequent Urination

What passing urine more often than usual means, the kidney, urinary, diabetes and other causes, and the assessment that follows.

Overview

Frequent urination means needing to pass urine more often than usual, with or without an increase in the total amount. It is a symptom rather than a diagnosis, and it has many causes — from a simple urinary infection to diabetes, an enlarged prostate, or a kidney-handling problem. This page covers the kidney and general-physician angle; where the cause is clearly in the urinary tract, care connects to the Urinary Problems service, and where it points to high blood sugar, to the Diabetes & Metabolic Health service.

Common causes

  • Urinary tract infection — often with burning or urgency.
  • Diabetes — high blood sugar draws extra water into the urine.
  • An enlarged prostate in older men.
  • Overactive bladder or bladder irritability.
  • High fluid intake, caffeine or alcohol.
  • Certain medicines, including water tablets.
  • Kidney-handling problems such as salt or water wasting.

Symptoms and associated features

The pattern matters. Passing large volumes each time suggests diabetes or a kidney-handling problem; passing small amounts with urgency or burning suggests infection; and waking repeatedly at night to pass urine may point to an enlarged prostate, heart failure or uncontrolled diabetes. Pain, fever, blood in the urine or swelling add further clues.

Evaluation and investigations

Evaluation includes urine routine and microscopy and a culture when infection is suspected, blood sugar or HbA1c, kidney function tests (creatinine and eGFR), electrolytes, and an ultrasound of the kidneys and bladder. A review of fluids, caffeine, alcohol and medicines is part of the assessment.

Management overview

Management is directed at the cause. A urinary infection is treated and the urine re-checked. Diabetes is managed with the appropriate sugar-control plan. An enlarged prostate, overactive bladder or kidney-handling problem is treated specifically. Simple measures — moderating evening fluids and caffeine — help when no disease is found.

Follow-up and monitoring

Follow-up depends on the cause; kidney function and urine are monitored where relevant. See Kidney Health Monitoring.

Warning signs — seek prompt care

  • Frequent urination with fever, chills or back pain.
  • Frequent urination with visible blood in the urine.
  • A sudden, large increase in urine output with thirst and weight loss.
  • Frequent urination with swelling or reduced urine output.
