Thyroid Medication Review

Overview

Many people take thyroid medication for years without a structured review, yet dose needs change with age, weight, pregnancy, menopause and other conditions. A medication review checks whether the current dose is correct, whether the tablet is being taken in a way that allows full absorption, and whether any lingering symptoms are truly thyroid-related or have another cause.

When a review is valuable

• Ongoing symptoms (fatigue, weight, mood, hair) despite “normal” blood results.
• A suppressed or raised TSH on the current dose.
• Weight change, pregnancy or menopause since the dose was last set.
• Starting medicines that interact with thyroid tablets (calcium, iron, antacids).
• Long-term use without a recent review.
• Switching between brands or formulations.

What a review covers

• Confirming the diagnosis the tablets were started for.
• Checking the dose against current TSH and symptoms.
• Reviewing timing — taken fasting, separate from other tablets and food.
• Identifying absorption issues and interactions.
• Considering whether a low TSH reflects over-replacement.
• Looking for non-thyroid causes of persistent symptoms.

Management

The dose may be adjusted up, down, or confirmed as correct; timing and interactions are corrected; and any non-thyroid cause of symptoms is addressed rather than chasing a number with more tablets. After a change, TSH is rechecked at the appropriate interval.
