Thyroid Disorders
Subclinical Hyperthyroidism
A mildly suppressed TSH with normal thyroid hormone levels — when it matters (especially for the heart and bones), and when monitoring is enough.

Overview
Subclinical hyperthyroidism means the TSH is slightly low or suppressed but the free T4 and free T3 are still within the normal range — the gland is mildly overactive but hormone levels have not yet risen. It may cause no symptoms, yet over time even a mild excess can affect the heart rhythm and bone density, so the decision to treat or monitor is based on more than the number alone.
This page focuses on the borderline overactive pattern. For the established condition, see Hyperthyroidism; for interpreting any abnormal result, see Abnormal Thyroid Function Tests.
When it matters most
- Older adults — increased risk of atrial fibrillation (irregular heartbeat) and bone thinning.
- Persistent suppression rather than a one-off low reading.
- Presence of palpitations, weight loss or osteopenia.
- TSH fully suppressed (below the detectable range) — more likely to warrant treatment.
Causes to consider
- Early or mild Graves’ disease or toxic nodular disease.
- Excess levothyroxine dosing — a common and reversible cause.
- Thyroiditis with temporary hormone release.
- Non-thyroid illness or certain medicines.
Evaluation
- Repeat TSH and free T4/free T3 after several weeks.
- Review current thyroid medication dose if applicable.
- Assess for palpitations, irregular heartbeat and bone health.
- ECG where cardiac symptoms are present.
Management and follow-up
If caused by too much levothyroxine, the dose is reduced and the TSH rechecked. If due to mild gland overactivity, treatment may be considered in older adults or those with cardiac/bone concerns, while younger, symptom-free people are often monitored with periodic testing.
How we help
