Thyroid Disorders
Thyroid-related Fatigue
When tiredness is genuinely linked to a thyroid disorder, when it has another cause, and how persistent fatigue is properly assessed.

Overview
Fatigue is the symptom most often blamed on the thyroid, and it is a real feature of both under- and overactive disease — in underactivity through low energy, in overactivity through a body that is constantly overdriven and exhausted. But tiredness is also one of the commonest symptoms in general practice, with anaemia, vitamin deficiencies, poor sleep, diabetes, depression and many other causes. The thyroid is therefore one part of a proper fatigue assessment, not the first assumption.
This page is about the fatigue symptom. For the conditions, see Hypothyroidism and Hyperthyroidism; for broader fatigue care, see Fatigue, Weakness & General Symptoms.
When fatigue may be thyroid-related
- Fatigue alongside other thyroid symptoms (weight change, temperature intolerance, palpitations).
- Fatigue with an abnormal TSH.
- Fatigue that persists despite a normal TSH on the correct levothyroxine dose — usually non-thyroid.
- New fatigue after a change in thyroid dose.
Other common causes to check
- Anaemia — iron deficiency is common and easily missed.
- Vitamin D and B12 deficiency.
- Diabetes or prediabetes.
- Poor sleep and sleep apnoea.
- Depression and chronic stress.
- Other hormonal conditions.
Evaluation
- TSH and free T4.
- Full blood count, iron studies, vitamin D and B12.
- Blood sugar and HbA1c.
- Review of sleep, mood and lifestyle.
Management
If the thyroid is abnormal, treating it correctly is the first step. If the thyroid is normal or already well controlled, attention turns to the other identifiable causes — anaemia, vitamins, sleep, mood and blood sugar — rather than adjusting a correct thyroid dose in pursuit of tiredness.
How we help
Dr. Nayak runs a structured fatigue assessment that includes but does not stop at the thyroid, treats any abnormality found, and avoids unnecessary thyroid-dose changes when the cause lies elsewhere. Consultations are available in-clinic in Bhubaneswar and over teleconsultation.
