Abnormal Thyroid Function Tests
A plain-language guide to reading thyroid blood-test results — what TSH, T4, T3 and antibodies tell you, and how abnormal patterns are interpreted together.

Overview
Thyroid function tests are among the most commonly ordered blood tests, and also among the most commonly misread. A result outside the reference range is not always disease, and a result within range does not always exclude a problem. The key is reading the tests together — TSH, free T4, free T3 and antibodies — and interpreting them in your clinical context rather than fixating on a single number.
This page is the interpretation guide. For specific abnormal patterns, see High TSH and Low TSH; for the conditions they point to, see Hypothyroidism and Hyperthyroidism.
What each test measures
- TSH — the pituitary’s signal; the most sensitive first-line marker of thyroid function.
- Free T4 — the main hormone actually produced by the thyroid; confirms the hormone level.
- Free T3 — the active hormone; checked in selected cases, especially suspected T3-toxicosis.
- Anti-TPO antibodies — marker of autoimmune thyroid disease (Hashimoto’s).
- TSH receptor antibodies — marker of Graves’ disease.
Common abnormal patterns
- High TSH, low T4 — overt hypothyroidism.
- High TSH, normal T4 — subclinical hypothyroidism.
- Low TSH, high T4/T3 — overt hyperthyroidism.
- Low TSH, normal T4/T3 — subclinical hyperthyroidism.
- Normal TSH with abnormal T4 — consider pituitary causes or assay interference.
- Mildly abnormal single result that normalises on repeat — transient fluctuation.
Why a single abnormal result is not enough
TSH naturally varies through the day and with illness, recovery, pregnancy and certain medicines. Reference ranges are population averages, so a value just outside the range may be normal for you. Confirming an abnormal result with a repeat test — and pairing TSH with free T4 — is standard before starting lifelong treatment.
How we help
