Chronic Cough

A cough lasting beyond eight weeks — the systematic causes to check (asthma, reflux, post-nasal drip, COPD, medicines) and how it is investigated.

Overview

A chronic cough is defined as one lasting beyond eight weeks in an adult. Unlike an acute cough, it is rarely self-limiting and almost always points to an underlying cause that needs to be identified and treated. The three most common causes in non-smoking adults are asthma (or eosinophilic airway inflammation), acid reflux, and post-nasal drip from nasal allergies — often called the “triad” of chronic cough. In smokers, COPD and the effects of smoking itself are leading considerations. This page covers the chronic context; for a cough lasting three to eight weeks see Persistent Cough.

Common causes

  • Asthma and eosinophilic airway inflammation (cough-variant asthma).
  • Gastro-oesophageal reflux disease (acid reflux).
  • Post-nasal drip from allergic rhinitis or sinusitis.
  • COPD and chronic bronchitis, especially in smokers.
  • ACE inhibitor medicines (a common blood-pressure drug class).
  • Tuberculosis and other chronic infections.
  • Lung fibrosis or, less commonly, a lung tumour — particularly in smokers with weight loss.

Differentiating chronic from persistent cough

A persistent (subacute) cough lasts three to eight weeks and most often follows a viral infection; it frequently settles without extensive tests. A chronic cough lasts beyond eight weeks and warrants a systematic work-up. The two are related but distinct — a persistent cough that crosses the eight-week mark becomes a chronic cough and is then investigated accordingly.

Evaluation

Evaluation reviews the cough character, triggers, smoking and medication history, and any reflux or nasal symptoms. A chest examination is performed, and tests are selected based on the leads — typically a chest X-ray and spirometry, with consideration of allergy testing, a trial of reflux treatment, review of blood-pressure medicines, and further imaging or bronchoscopy when needed.

Management

Management treats the identified cause: inhalers for asthma, anti-reflux treatment and lifestyle measures for reflux, nasal sprays and antihistamines for post-nasal drip, switching an ACE inhibitor for medicine-related cough, and smoking cessation with bronchodilators for COPD. Simply suppressing a chronic cough with cough syrup without finding the cause is avoided.

Warning signs — seek urgent care

  • Coughing up blood.
  • Unexplained weight loss, night sweats or persistent fever (possible tuberculosis or tumour).
  • New or worsening breathlessness.
  • A chronic cough in a long-term smoker that changes in character.
