Respiratory & Breathing Problems
Chest Congestion
A feeling of heaviness or mucus in the chest — common in bronchitis, infections and COPD, what it means, and when it needs assessment.

Overview
Chest congestion is the sensation of heaviness, fullness or mucus in the chest, often with a productive cough and sometimes wheeze. It is a feature of acute bronchitis, respiratory infections, COPD and asthma flare-ups, rather than a diagnosis in itself. The aim is to identify the underlying cause and treat it. This page covers the symptom; related pages cover the specific conditions.
Common causes
- Acute bronchitis and viral respiratory infections.
- Pneumonia and lower respiratory infections.
- COPD and chronic bronchitis.
- Asthma flare-ups with mucus.
- Post-nasal drip draining into the chest.
Evaluation
Evaluation reviews the duration, phlegm character, fever, breathlessness and smoking history. A chest examination is performed, and a chest X-ray is ordered when pneumonia or another lower respiratory infection is suspected, with sputum tests and blood tests as needed.
Management
Management treats the underlying cause: supportive care for viral bronchitis, antibiotics for bacterial infection, inhalers for asthma or COPD, and hydration to help clear mucus. Steam inhalation and warm fluids can ease the sensation symptomatically.
Warning signs — seek urgent care
- Breathlessness, chest pain or bluish lips.
- High fever with confusion.
- Blood in the phlegm.
- Worsening despite treatment.
