Digestive Problems
Chronic Constipation
Constipation that persists over weeks or months — looking for underlying causes, structured management, and when further tests are needed.

Overview
Chronic constipation is constipation that persists over weeks or months despite reasonable lifestyle measures. Rather than treating it as a recurring nuisance, a structured review looks for an underlying cause — low fibre or fluids, low activity, medicines, an underactive thyroid, diabetes, or a disorder of the bowel’s muscle and nerve function. This page covers the chronic context; general constipation and its simple measures are on the Constipation page, which links here.
Common underlying causes
- Persistently low fibre or fluid intake.
- Low physical activity or prolonged immobility.
- Regular use of certain medicines, including painkillers and some blood-pressure drugs.
- An underactive thyroid (hypothyroidism).
- Diabetes, which can affect gut nerve function.
- Disorders of pelvic-floor or bowel-coordination function.
- A structural problem in the lower bowel, in some cases.
Evaluation and investigations
Evaluation reviews the duration, stool form, straining, and any warning features, along with medicines and existing conditions. Blood tests may check thyroid function and blood sugar. Further tests, including a colonoscopy or specialised bowel-function tests, are arranged when there are warning features or when the pattern suggests a structural or functional disorder.
Management
Management combines structured lifestyle measures — adequate fibre, fluids and activity — with sensible use of laxatives and treatment of any underlying cause such as thyroid disease or diabetes. Long-term stimulant-laxative reliance is avoided where possible, and treatment is reviewed rather than continued indefinitely without reassessment.
Warning signs — seek prompt care
- A recent change in bowel habit with constipation.
- Blood in the stool or black, tarry stools.
- Severe abdominal pain or persistent bloating.
- Unexplained weight loss.
- Alternating constipation and diarrhoea.
