Change in Bowel Habits

A persistent change in your usual bowel pattern — why it should not be ignored, the causes it may point to, and when to investigate.

Overview

A change in bowel habits means a persistent shift from your usual pattern — stools becoming consistently looser, more frequent, less frequent, or different in form or calibre. While brief changes after an infection or a change in diet are common, a change that persists beyond a few weeks should not be ignored, especially when it comes with bleeding, mucus, weight loss or night symptoms. This page frames the symptom and the reasons it warrants review.

What counts as a meaningful change

  • Stools consistently looser or more frequent than your usual.
  • Stools consistently harder or less frequent (a new constipation pattern).
  • Alternating diarrhoea and constipation.
  • A change in stool shape or calibre, such as pencil-thin stools.
  • Persistent mucus in the stool.
  • A change that persists beyond a few weeks.

Possible causes

  • Irritable bowel syndrome (IBS).
  • A recent or lingering gut infection.
  • Inflammatory bowel disease.
  • Dietary changes or food intolerance.
  • Medicines.
  • Thyroid disorders or diabetes.
  • In some cases, a structural problem in the bowel, including bowel cancer — hence review.

Evaluation and investigations

Evaluation reviews the nature and duration of the change and any accompanying features. Blood tests, a stool examination (including for hidden blood) and, when indicated, a colonoscopy are arranged to identify the cause. The aim is to distinguish a benign cause such as IBS from a condition needing specific treatment.

Warning signs — seek prompt care

  • A persistent change lasting beyond a few weeks.
  • Blood in the stool or black, tarry stools.
  • Unintended weight loss.
  • Persistent abdominal pain or bloating.
  • A new onset of symptoms in older age.
  • A family history of bowel cancer or inflammatory bowel disease.
