Respiratory & Breathing Problems

Wheezing

A whistling chest sound from narrowed airways — typical of asthma and COPD, but also bronchitis and allergies, and when it needs urgent care.

Person using a reliever inhaler with a stethoscope and a peak flow meter on a clinic table in Bhubaneswar — Wheezing

Overview

Wheezing is a high-pitched whistling sound made when air moves through narrowed airways. It is most often heard on breathing out and is a hallmark of asthma and COPD, though it can also occur with acute bronchitis, allergic airway irritation, or a respiratory infection. Wheezing that is new, worsening, or accompanied by breathlessness needs assessment to find and treat the underlying cause. Severe wheezing with marked breathlessness can be an emergency.

Common causes

  • Asthma — the classic cause; often with triggers and a fluctuating course.
  • COPD and chronic bronchitis — especially in smokers.
  • Acute bronchitis and viral respiratory infections.
  • Allergic airway inflammation.
  • A flare of existing airway disease triggered by infection, dust or pollution.

Evaluation

Evaluation reviews the triggers, frequency, timing and any existing diagnosis. A chest examination is performed, and spirometry (lung function) is the key test to confirm asthma or COPD and measure severity. A chest X-ray and blood tests are ordered when infection or another cause is suspected.

Management

Management treats the underlying airway disease: reliever and preventer inhalers for asthma, bronchodilator inhalers and smoking cessation for COPD, and supportive care for viral bronchitis. Trigger avoidance and an action plan for flare-ups are part of every plan.

Warning signs — seek urgent care

  • Severe wheezing with breathlessness at rest or inability to speak full sentences.
  • Bluish lips or very low oxygen.
  • A severe asthma attack not responding to the reliever inhaler.
  • Wheezing with chest pain, fainting or confusion.
