Overview

Allergic respiratory symptoms include sneezing, a runny or blocked nose, itchy eyes, cough and sometimes wheeze, caused by the immune system over-reacting to allergens such as dust mites, pollen, animal dander or mould. Allergic rhinitis (hay fever) is the commonest form, and there is a strong link between nasal allergies and asthma — many people have both. Effective treatment controls symptoms and may reduce asthma flare-ups. This page covers the allergic pattern; for wheeze and the disease itself see Wheezing and Asthma.

Common symptoms

  • Sneezing, runny or blocked nose, and itchy nose.
  • Itchy, watery or red eyes.
  • A dry cough, often worse at night or with allergen exposure.
  • Wheeze or breathlessness in those with asthma.
  • Symptoms that are seasonal or flare with specific exposures.

Common triggers

  • Dust mites and house dust.
  • Pollen from grasses, trees and weeds.
  • Animal dander.
  • Mould and damp.
  • Cockroach and insect allergens.
  • Air pollution and smoke.

Evaluation

Evaluation reviews the symptom pattern, triggers and any link to asthma. Allergy testing (skin-prick or blood tests) may identify specific triggers, and spirometry is done when asthma is suspected. The link between allergic rhinitis and asthma is specifically checked, as treating the nose often helps the chest.

Management

Management combines allergen avoidance, nasal steroid sprays, antihistamines, and eye drops for eye symptoms. Where asthma coexists, inhalers are optimised. For severe allergies, specific immunotherapy may be considered. Reducing exposure to dust, pets and mould is a practical first step.

Warning signs — seek urgent care

  • Severe breathlessness or wheeze not responding to usual treatment.
  • Swelling of the lips, tongue or throat with difficulty breathing (anaphylaxis) — emergency.
  • A severe asthma attack.
