Fever with Headache

Overview

Headache with fever is common and usually part of a viral illness, dengue, flu or malaria. However, a severe headache with fever can occasionally signal meningitis or another serious infection. Common causes

• Viral fever, influenza and dengue — often a moderate-to-severe headache.
• Malaria — fever with chills and headache.
• Typhoid — sustained fever with headache.
• COVID-19 and other respiratory viral illness.
• Meningitis — severe headache with stiff neck, fever, vomiting and confusion (emergency).
• Sinus infection — fever with facial pain and headache.

When a fever with headache is an emergency

• Severe headache with a stiff neck, vomiting and sensitivity to light — possible meningitis.
• Headache with confusion, drowsiness or seizures.
• Sudden severe (“thunderclap”) headache with fever.
• Headache with rash or bleeding.

Evaluation

For ordinary viral-pattern headache with fever, tests may be limited. When dengue or malaria is suspected, the relevant blood tests are ordered. Any emergency features prompt urgent hospital assessment rather than a routine visit.

Management

Most cases are managed with rest, fluids and fever-reducing medicine while the underlying infection is treated. Emergency features require immediate hospital care.
