Overview

Nocturia means waking one or more times at night to pass urine. Occasional nocturia is common, especially with age, but regular waking two or more times a night disturbs sleep and quality of life and may signal an underlying condition. It becomes more frequent with age but is not always benign — new or worsening nocturia deserves evaluation rather than being dismissed as a normal part of ageing.

Common causes

  • High fluid, caffeine or alcohol intake in the evening.
  • An enlarged prostate in older men, causing night-time frequency.
  • Uncontrolled diabetes, drawing extra water into the urine through the night.
  • Overactive bladder.
  • Heart failure, in which fluid retained during the day is passed at night.
  • Kidney-handling problems that affect night-time urine concentration.
  • Certain medicines, particularly water tablets taken late in the day.
  • Sleep disturbance, where waking for another reason leads to passing urine.

Symptoms and associated features

The defining feature is waking from sleep to pass urine. Associated features guide the cause: daytime frequency and a weak stream suggest prostate obstruction; great thirst and weight loss suggest diabetes; ankle swelling and breathlessness suggest heart failure; and burning or cloudiness suggest infection. A simple fluid diary — recording what is drunk and passed over 24 hours — is often revealing.

Evaluation and investigations

Evaluation reviews the pattern, evening habits, medicines and existing conditions, and uses a fluid diary where helpful. Tests typically include urine routine and microscopy, blood sugar or HbA1c, kidney function tests, and an ultrasound of the kidneys and bladder. Prostate assessment is considered in men; heart and ankle examination when heart failure is suspected. See Frequent Urination and Excessive Urination / Polyuria.

Management overview

Management is directed at the cause. Simple measures — reducing evening fluids, caffeine and alcohol, and adjusting the timing of water tablets — help many people. An enlarged prostate, diabetes, overactive bladder or heart failure is treated specifically. When no disease is found, behavioural measures alone often restore sleep.

Follow-up and monitoring

Follow-up reviews the fluid diary and symptom response. Persistent nocturia despite treatment is reassessed for an underlying cause and referred for specialist care when appropriate.

Warning signs — seek prompt care

  • Nocturia with great thirst and weight loss — check blood sugar for diabetes.
  • Nocturia with ankle swelling and breathlessness — needs assessment for heart failure.
  • Nocturia with visible blood in the urine.
  • Nocturia with burning and fever — possible infection.
