Swelling & Oedema
Swelling with High Blood Pressure
Swelling alongside high blood pressure — why the combination points to the kidneys or heart, the evaluation, and when it needs prompt attention.

Overview
Swelling alongside high blood pressure is a combination that often points to the kidneys or the heart. Kidney disease can both raise blood pressure and cause swelling; heart failure can cause both leg swelling and raised blood pressure; and some blood-pressure medicines themselves can cause ankle swelling. The combination is assessed rather than treated as two separate issues. This page covers the combined presentation; the management of blood pressure is covered under Hypertension & Cardiovascular Risk, and kidney-related swelling under Kidney Health.
Common possible causes
- Kidney disease — swelling with raised blood pressure, often with foamy urine or abnormal tests.
- Heart failure — leg swelling with breathlessness and raised blood pressure.
- Certain blood-pressure medicines — calcium-channel blockers can cause ankle swelling.
- Coexisting diabetes and kidney involvement.
- Resistant or poorly controlled hypertension with kidney effects.
Symptoms and associated features
Swelling with foamy urine and abnormal kidney tests points to a kidney cause; swelling with breathlessness on exertion or lying flat points to heart failure; swelling that appeared after a blood-pressure medicine was started or changed points to a medication effect; and swelling with a very high blood pressure and headache or blurred vision needs prompt attention.
Evaluation and investigations
Evaluation combines a history and examination with targeted tests — kidney function, urine ACR, serum albumin, complete blood count, blood sugar, a heart marker and ECG where indicated, and a review of current blood-pressure medicines. See Hypertension & Cardiovascular Risk, Kidney Health and Abnormal Test Results.
Management overview
Management treats the cause. Kidney disease is managed to protect function and control blood pressure, often with specific medicines that also reduce protein leak; heart failure is managed with heart medication; and medication-related swelling is addressed by medical review and adjustment — medicines are not stopped independently. Blood-pressure control is optimised as part of the plan.
Follow-up and monitoring
Follow-up confirms the swelling is settling and blood pressure is controlled, with periodic re-checking of kidney function, urine ACR and blood pressure. Persistent or worsening swelling is reviewed and referred for specialist care when appropriate.
Warning signs — seek urgent care
- A very high blood pressure with severe headache, blurred vision or chest pain — urgent.
- Swelling with breathlessness, especially on lying flat — urgent.
- Swelling with rapidly reduced urine output — urgent.
- Swelling with chest pain or fainting — emergency.
