Awareness of Heartbeat

Noticing your own heartbeat when you normally would not — usually benign, how it is assessed, and when it warrants further evaluation.

Overview

Awareness of heartbeat is the sensation of feeling your heart beat in your chest, throat or neck when you would not normally notice it. It is very common and, in most people, is simply the result of a quiet environment, lying down, anxiety, caffeine, tiredness or a mild illness, with a healthy heart underneath. It becomes a reason to assess when it is new, persistent, distressing, or accompanied by other symptoms such as a fast, irregular or skipped beat, chest discomfort, dizziness or breathlessness.

Common causes

  • Benign triggers — quiet surroundings, lying down, anxiety, caffeine, tiredness and poor sleep.
  • Stress and heightened body awareness.
  • Ectopic beats and minor rhythm variations.
  • An over-active thyroid.
  • Anaemia, which can make the heartbeat more forceful and more noticeable.
  • Fever and dehydration.
  • Medication effects.

Symptoms and associated features

The hallmark is noticing the heartbeat itself, often at rest or in bed, without it necessarily being fast or irregular. Isolated awareness in an otherwise well person is usually benign. Awareness with a fast, irregular or skipped pattern, or with chest discomfort, dizziness or breathlessness, is assessed further.

Evaluation and investigations

Evaluation combines a history and examination with an ECG and targeted blood tests — thyroid function, complete blood count for anaemia, and electrolytes. Further testing such as a Holter monitor is arranged only when the pattern warrants it. See ECG Evaluation for Symptoms and Abnormal Test Results.

Management overview

Management treats the cause. Benign awareness is managed with reassurance, reducing caffeine, improving sleep and addressing stress; an over-active thyroid or anaemia is treated specifically; and any rhythm problem is managed accordingly.

Follow-up and monitoring

Follow-up confirms the awareness has settled or is stable. New or changing features are reviewed and referred for specialist assessment when appropriate.

Warning signs — seek urgent care

  • Awareness of heartbeat with chest pain, dizziness, fainting or breathlessness — urgent.
  • A fast or irregular pattern that does not settle — urgent.
  • Awareness with collapse or loss of consciousness — emergency.
