Skipped Heartbeat Sensation
The feeling that the heart skips or adds a beat — usually benign ectopic beats, how they are assessed, and when they need further evaluation.

Overview
A skipped heartbeat sensation is the feeling that the heart has missed a beat, paused, or added an extra thump. It is most often caused by ectopic beats — extra beats arising from the upper or lower chambers of the heart — followed by a brief pause that feels like a “skip”. In an otherwise healthy heart these are common and harmless, often noticed at rest or in bed. They become a reason to assess further when they are very frequent, come with other symptoms, or occur in someone with underlying heart disease.
Common causes
- Ectopic (extra) beats — the commonest cause, usually benign.
- Triggers — caffeine, stress, anxiety, poor sleep, alcohol and nicotine.
- An over-active thyroid.
- Electrolyte disturbances such as low potassium or magnesium.
- Medication effects.
- Underlying heart disease, which makes ectopics more significant.
Symptoms and associated features
The typical feeling is a brief pause or a thump after a normal beat, often more noticeable when lying quietly. Infrequent skipped beats in an otherwise well person are usually benign. Frequent skipped beats, a sensation of irregularity that does not settle, or skipped beats with chest discomfort, dizziness or breathlessness, warrant assessment.
Evaluation and investigations
Evaluation combines a history, examination and an ECG. Blood tests check thyroid function, electrolytes and blood sugar. When the sensation is intermittent, an ambulatory ECG (Holter monitor) captures the beats and quantifies how frequent they are. See ECG Evaluation for Symptoms and Abnormal Test Results.
Management overview
Management treats the cause. Benign ectopic beats in an otherwise normal heart often need no treatment beyond reassurance and reducing triggers such as caffeine and stress; an over-active thyroid or electrolyte disturbance is corrected; and ectopics in the setting of underlying heart disease are managed with specialist input.
Follow-up and monitoring
Follow-up confirms the sensation has settled or is stable. Persistent, frequent ectopics are reviewed and referred for specialist cardiac assessment when appropriate.
Warning signs — seek urgent care
- Frequent skipped beats with chest pain, dizziness, fainting or breathlessness — urgent.
- Skipped beats with a known heart condition — assessment rather than reassurance alone.
- A sudden irregular fast rhythm that does not settle — urgent.
