Irregular Heartbeat Sensation
The sensation of the heart beating out of rhythm — common benign ectopic beats versus the more important atrial fibrillation, how it is evaluated, and when it needs prompt attention.

Overview
An irregular heartbeat sensation is the feeling that the heart is out of rhythm — with pauses, jumps, an uneven pattern, or beats that feel added or dropped. It is very commonly caused by occasional ectopic beats, which are usually harmless. A persistently irregular pulse, however, can indicate atrial fibrillation — an important rhythm problem that raises the risk of stroke and needs specific management. Distinguishing the two is the main reason to assess an irregular sensation rather than dismiss it.
Common causes
- Ectopic (extra) beats — the commonest, usually benign cause.
- Atrial fibrillation — a persistently irregular rhythm that needs assessment and stroke-risk management.
- Other arrhythmias, including intermittent rhythm problems.
- Benign triggers — caffeine, stress, poor sleep and alcohol.
- An over-active thyroid, which can trigger an irregular rhythm.
- Electrolyte disturbances and medication effects.
Symptoms and associated features
Occasional skipped or added beats at rest in an otherwise well person usually point to ectopics. A continuously irregular pulse, an irregular sensation with dizziness or breathlessness, or episodes that come and go frequently, raise the need for a proper rhythm assessment. A history of high blood pressure, heart disease, diabetes or a thyroid disorder adds weight to the assessment.
Evaluation and investigations
Evaluation combines a careful history, a pulse check and an ECG. Blood tests check thyroid function, electrolytes and blood sugar. Because atrial fibrillation can be intermittent, an ambulatory ECG (Holter monitor) is often used when the ECG is normal between episodes. See ECG Evaluation for Symptoms and Abnormal Test Results.
Management overview
Management treats the cause. Benign ectopic beats in an otherwise normal heart often need no treatment beyond reassurance and reducing triggers; atrial fibrillation is managed with rate or rhythm control and an assessment of stroke risk that may point to blood-thinning medication; and underlying triggers such as a thyroid disorder are treated specifically.
Follow-up and monitoring
Follow-up confirms the rhythm is controlled and, where atrial fibrillation is present, that stroke-risk management is in place. Persistent or recurrent irregular sensations are reviewed and referred for specialist cardiac assessment when appropriate.
Warning signs — seek urgent care
- An irregular heartbeat with chest pain, dizziness, fainting or breathlessness — urgent.
- A sudden irregular fast rhythm that does not settle — urgent.
- Irregular heartbeat with weakness on one side of the body or slurred speech — possible stroke; emergency.
How we help
Dr. Nayak evaluates an irregular heartbeat sensation with a structured history, pulse check, ECG and targeted blood tests, arranges a Holter monitor where needed, and manages the underlying cause or refers for specialist cardiac care.
