Breathlessness Due to Anaemia
When breathlessness is caused by low haemoglobin — how it behaves, how it is distinguished from heart and lung causes, and how it improves.

Overview
Breathlessness is a common symptom of anaemia. With less haemoglobin to carry oxygen, the body raises the breathing rate to compensate, so breathlessness appears first with activity and, when the anaemia is severe, even at rest. This page focuses on breathlessness as a symptom of anaemia; the broader evaluation of breathlessness (which includes heart and lung causes) is on the Respiratory & Breathing Problems page.
How anaemia-related breathlessness behaves
- Worse with exertion, easing with rest, in the early stages.
- Often accompanied by tiredness, palpitations and dizziness.
- Improves as the haemoglobin rises with correct treatment.
- When severe, can occur at rest or on minimal activity.
Distinguishing it from heart and lung causes
Breathlessness also comes from the heart (heart failure, rhythm problems) and the lungs (asthma, COPD, infection, clots). Anaemia-related breathlessness is usually proportionate to the haemoglobin level and improves with correction, while heart or lung breathlessness has its own patterns — wheeze, orthopnoea, chest pain, cough. A blood count, examination and, when needed, an ECG and chest assessment separate the causes. See Respiratory & Breathing Problems and Palpitations & Cardiovascular Symptoms.
Evaluation and investigations
A complete blood count confirms anaemia and its pattern; iron studies, B12 and folate, and kidney and thyroid function are added as needed. When heart or lung disease is possible, an ECG, chest examination and imaging or breathing tests are arranged. See Anaemia and Abnormal Complete Blood Count (CBC).
Management overview
Breathlessness from anaemia improves as the underlying cause is treated. If a heart or lung cause is found, it is managed alongside or instead. Severe, symptomatic anaemia may need urgent treatment or transfusion. See Iron Deficiency Anaemia and Nutritional Anaemia.
Warning signs — seek prompt care
- Breathlessness at rest, chest pain or fainting with a very low haemoglobin — emergency.
- Sudden breathlessness — possible a lung or heart cause rather than anaemia.
