High Resting Heart Rate
A persistently fast heartbeat at rest found on measurement or a fitness tracker — the medical causes from thyroid and anaemia to fitness and stress, the evaluation, and when it needs attention.

Overview
A high resting heart rate is a persistently elevated heartbeat at rest — often noticed on a fitness tracker, a blood-pressure monitor, or a routine check — rather than the episodic sensation of the heart racing. While a resting rate is influenced by fitness, stress and caffeine, a consistently high resting rate can reflect an underlying cause such as a thyroid disorder, anaemia, fever, dehydration, or a medication effect, and occasionally a rhythm problem. This page focuses on the resting rate as a finding; the episodic sensation of a fast heartbeat is covered on Fast Heartbeat.
Common causes
- Low cardiovascular fitness and deconditioning.
- Stress, anxiety and poor sleep.
- Caffeine, nicotine and energy drinks.
- An over-active thyroid (hyperthyroidism).
- Anaemia and low haemoglobin.
- Fever, infection and dehydration.
- Medication effects — inhalers and certain other medicines.
- Pain and chronic illness.
- A persistently fast rhythm such as inappropriate sinus tachycardia.
Symptoms and associated features
The finding is a resting rate consistently above the expected range. Associated features guide the cause: weight loss and heat intolerance suggest an over-active thyroid; tiredness and pallor suggest anaemia; fever suggests infection; and a sensation of the heart racing, dizziness or breathlessness suggests a rhythm problem. A single high reading during stress or illness is less concerning than a persistently high rate when well.
Evaluation and investigations
Evaluation combines a history and examination with an ECG and blood tests — thyroid function, complete blood count for anaemia, electrolytes, and blood sugar. A Holter monitor is arranged when a rhythm problem is suspected. See ECG Evaluation for Symptoms, Thyroid Disorders and Abnormal Test Results.
Management overview
Management treats the cause. Fitness and lifestyle measures improve a deconditioned rate; an over-active thyroid or anaemia is treated; fever and dehydration are addressed; medication effects are reviewed; and any rhythm problem is managed specifically.
Follow-up and monitoring
Follow-up confirms the resting rate is settling and any underlying cause is controlled. A persistently high rate despite treatment is reviewed and referred for specialist assessment when appropriate.
Warning signs — seek urgent care
- A very high resting rate with chest pain, dizziness, fainting or breathlessness — urgent.
- A high rate with a rapid irregular pulse — urgent.
- A high rate with collapse or loss of consciousness — emergency.
