Persistent Swelling

Swelling that does not settle over weeks — the chronic causes, how it is evaluated, and when persistent swelling needs prompt attention.

Overview

Persistent swelling is fluid accumulation that lasts weeks or longer rather than settling in days. It usually reflects a chronic underlying cause — venous insufficiency, heart failure, kidney or liver disease, an under-active thyroid, or a long-term medicine. Persistent swelling is assessed even when it is mild and not rapidly worsening, because identifying the cause early prevents progression. This page covers the persistent pattern; the cause-specific pages are linked below.

Common possible causes

  • Venous insufficiency and varicose veins — the commonest chronic cause.
  • Chronic heart failure.
  • Chronic kidney disease.
  • Chronic liver disease with low protein.
  • An under-active thyroid.
  • Long-term medicines — calcium-channel blockers, steroids, certain others.
  • Low protein from heavy urine leak or nutritional causes.

Symptoms and associated features

Persistent both-legged swelling that worsens through the day and eases overnight is often venous. Features that warrant assessment include swelling that no longer eases overnight, breathlessness, foamy urine, reduced urine, abdominal fullness, weight change, and skin changes or ulcers from long-standing venous swelling.

Evaluation and investigations

Evaluation combines a history and examination with targeted tests — kidney function, urine ACR, serum albumin and liver function, complete blood count, thyroid function, blood sugar, a heart marker where indicated, and an abdominal ultrasound. See Kidney Health, Liver & Fatty Liver Health and Abnormal Test Results.

Management overview

Management treats the underlying cause. Venous insufficiency is managed with elevation, movement, compression socks and skin care; heart, kidney and liver disease are treated specifically; a thyroid disorder is corrected; and medication-related swelling is addressed by medical review and adjustment.

Follow-up and monitoring

Follow-up confirms the swelling is settling and the underlying cause is controlled, with periodic re-checking of the relevant tests. Persistent swelling despite treatment is reviewed and referred for specialist care when appropriate.

Warning signs — seek urgent care

  • Persistent swelling that suddenly worsens or becomes one-sided — urgent.
  • Persistent swelling with new breathlessness — urgent.
  • Persistent swelling with reduced urine output — urgent.
  • A swollen leg with skin breakdown, redness or heat — assess promptly.
