Swelling & Oedema
Generalised Body Swelling
Widespread swelling of the face, limbs and abdomen — the systemic causes, how it is evaluated, and why it needs prompt assessment.

Overview
Generalised body swelling is widespread fluid accumulation affecting the face, limbs and often the abdomen, and sometimes accompanied by breathlessness from fluid around the swelling. It points to a systemic cause rather than a local one — most often kidney disease with heavy protein loss (nephrotic syndrome), heart failure, or liver disease with low blood protein. Because generalised swelling usually reflects a significant underlying condition, it is assessed promptly. This page covers the generalised pattern; the component patterns are explored on Facial Swelling, Leg Swelling and Fluid Retention.
Common possible causes
- Kidney disease with heavy protein leak (nephrotic syndrome) — swelling with foamy urine and low blood protein.
- Heart failure — generalised swelling with breathlessness, worse on exertion or lying flat.
- Liver disease — low blood protein with abdominal fluid and sometimes jaundice.
- An under-active thyroid.
- Severe nutritional low protein.
- Medicines, including steroids and certain others.
Symptoms and associated features
Swelling of the face on waking, the legs through the day, and a full abdomen, often with weight gain, points to generalised fluid retention. Breathlessness, reduced urine output, foamy urine, jaundice, tiredness and poor appetite are important accompanying features that guide the cause.
Evaluation and investigations
Evaluation combines a history and examination with targeted tests — kidney function, urine ACR, serum albumin and total protein, liver function, complete blood count, thyroid function, blood sugar, a heart marker (BNP/NT-proBNP), ECG and echocardiogram where indicated, and an abdominal ultrasound. See Kidney Health, Liver & Fatty Liver Health and Abnormal Test Results.
Management overview
Management treats the underlying cause. Kidney disease with protein leak is managed to protect function and reduce the leak; heart failure is managed with heart medication and fluid measures; liver disease is treated for the underlying cause; a thyroid disorder is corrected; and nutritional and medication causes are addressed. Fluid tablets may be used under medical supervision when appropriate.
Follow-up and monitoring
Follow-up confirms the swelling is settling and the underlying cause is controlled, with periodic re-checking of the relevant tests. Persistent or worsening generalised swelling is reviewed and referred for specialist care when appropriate.
Warning signs — seek urgent care
- Generalised swelling with breathlessness, especially on lying flat — urgent.
- Generalised swelling with rapidly reduced urine output — urgent.
- Swelling with chest pain, fainting or a rapid irregular pulse — emergency.
- Swelling with drowsiness or confusion — emergency.
