Eyelid Swelling

Puffy or swollen eyelids — the causes from allergy and thyroid to kidney and local eye problems, how it is evaluated, and when it needs attention.

Overview

Eyelid swelling is puffiness around the eyes, often most obvious in the morning. It can be a local eye or skin problem (allergy, stye, infection, contact-irritation), a sign of an under-active thyroid, or — when it accompanies leg swelling and foamy urine — a sign of a kidney cause. Isolated intermittent eyelid puffiness in an otherwise well person is often benign, but persistent or worsening puffiness is assessed. This page covers eyelid swelling; broader facial puffiness is explored on Facial Swelling.

Common possible causes

  • Local allergy — eye make-up, contact lenses, dust or pollen, often with itching.
  • A stye, chalazion or eyelid infection, usually with a local tender lump or redness.
  • An under-active thyroid — puffiness with dry skin and tiredness.
  • A kidney cause — morning eyelid puffiness with leg swelling and foamy urine.
  • Tiredness, crying, high salt intake and fluid retention.
  • Skin conditions such as contact dermatitis.

Symptoms and associated features

Itching points to allergy; a local tender red lump points to a stye or infection; puffiness with dry skin, weight gain and cold intolerance points to an under-active thyroid; and morning puffiness with leg swelling and foamy urine points to a kidney cause. Puffiness that is bilateral and persistent, or that appears with other swelling, is assessed rather than attributed to tiredness alone.

Evaluation and investigations

Evaluation combines a history and focused examination with targeted tests when a systemic cause is suspected — kidney function, urine ACR, serum albumin, and thyroid function. Local eye problems are assessed on examination. See Kidney Health, Thyroid Disorders and Abnormal Test Results.

Management overview

Management treats the cause. Local allergy is managed with avoidance and antihistamines; a stye or infection is treated with warm compresses and, where needed, antibiotics; an under-active thyroid is treated with levothyroxine; and a kidney cause is managed to protect function and reduce protein leak.

Follow-up and monitoring

Follow-up confirms the puffiness is settling and any underlying cause is controlled. Persistent or recurrent eyelid swelling is reviewed and referred for specialist care when appropriate.

Warning signs — seek urgent care

  • Eyelid swelling with redness, pain and reduced vision — possible eye infection; urgent.
  • Eyelid swelling with facial and lip swelling and breathing difficulty — possible severe allergy; emergency.
  • Persistent morning eyelid puffiness with leg swelling and foamy urine — assess for a kidney cause.
